MMOVE ON AND MOVE IN - PILOT RESTTLEMENT SERVICE Liverpool Supporting People programme has commissioned a pilot resettlement service. The pilot resettlement service is a partnership between support providers, housing associations and Supporting People. The pilot resettlement service offers a co-ordinated approach to the support and accommodation needs of service users who are ready to move on. The pilot delivers a programme of floating support, aimed at supporting service users to sustain a tenancy. By providing service users with the support services that are appropriate to their needs, the pilot aims to enable service users to access permanent independent accommodation. The provision of support services allows service users to be equipped with the skills and knowledge that are necessary to be able to sustain such accommodation. MENTAL HEALTH SUPPORTED HOUSING - Liverpool City Council Supported accommodation service comprising of 15 schemes across Liverpool. Staff support is provided by the Social Care Service of Mersey Care NHS Trust and tenancies are with a number of landlords mainly RSLs. There is a mix of self contained grouped flats and schemes with shared living areas. One scheme is for short medium stay and move on. In several schemes there is 24 hour staff support including sleepin availability overnight in others there is floating support. There are 83 places in all across the network. Supported Housing (shared or self contained) 8 units Accommodation Based Service. Accepts self referrals. Contact the number listed Alternative contact PPOWERHOUSE FOYER - RIVERSIDE Foyer offering supported accommodation for 16 -25 year olds. The service is staffed 24 hours per day. The entry criteria is you must be aged 16 to 25 years old, homeless or in danger of becoming homeless and you must commit to a personal development plan which may include taking part in life skills support sessions. Project Officers provide one-to-one support to individuals and this is through key working and personal development planning which is reviewed monthly. They and other members of the staff can offer information and advice in gaining access to other agencies, training and employment. There is a Resettlement Officer who will assist you in finding move-on accommodation after a successful stay at the foyer. The accommodation can house 52 young people in 12 single bedsit and 20 shared bedsits. There are 4 flats suitable for people with disabilities. RIVERSIDE HOUSING SSHELTERED -RADLEYS COURT - CDS CDS own seven sheltered schemes in Liverpool which offer independent living in secure surroundings. The accommodation is usually one or two bedrooms with fitted kitchen combined bathroom/toilet and a living room. All accommodation is centrally heated. Most of the schemes have lifts to all floors laundry facilities guest room for visitors communal lounge for social events residential TV licence for £5 and a public telephone. A community alarm service is provided 24 hours each day in case of emergency and for scheme staff to carry out daily welfare checks with residents. Each resident will have a needs and risk assessment carried out to ensure appropriate support is provided to facilitate independent living for as long as possible.
